I'm not exactly sure how to word a letter requestin gold addresses removed. I search the sample letters forum and came upon nothing. Can someone help me on this?

It really sort of depends. In my expierance, you can just dispute invalid address online with Trans Union (TU) and Equifax (EQ) with good results. As fro Experian, it's almost not worth the effort but we all try anyway. Just list each address and indicate that it is invalid. If you have any tradelines on your report that list that address, chances are you won't be able to get it off EX with a jackhammer.

Point is, there is no specific wording that is required in your letter other that the address is 'invalid'.

So, all i have to do is list all the addresses that are there and say "please remove these they are invalid"?

Does it matter that they are actual former addresses?

Also, under my SS# variations they have my NY drivers license # listed. Should I dispute this as well?

I just did that with Experian

I printed out the list of addresses they had on file for me and marked each one as valid or not valid. Same for phone numbers. Most (not all) were corrected. Those that remain are probably not worth the effort.

I disputed online, they gave me an 'invalid' option right on their form. If they are former address then legaly they can be reported. Many people recommend they be removed. Simply requesting removal usually works. If you have a whole lot of them, you may wish to only dispute 3-4 at a time.

As for the Dr. Lic number I cannot give you any advice. Mine does not appear on the report. I think if it did I would request to have it removed but I wouldn't dispute it as invalid. That and your passport are about the most widely accepted forms of Identification and I wouldn't want to imagine what kind of weird stuff could eventually happen if you claim that it's not your ID.

I sent you a quick PM with some info specific to issues in NY. They don't apply to your particular situation right here but you may find them helpful down the road. Good Luck.

Ok TU dispute old addresses done. When I go to Equifax I apparently need a EQ CR (I get the tri merge from TrueCredit)...I haven't been applying for credit so no denials. Any other way to get one or do I need to mail my dispute in?
