If it ever goes to trial, make damned sure they don't admit that affidavit against you - demand the right to cross-examine the records custodian, and if they don't appear, get the records thrown out.

Unless Ohio small claims procedure is unusually wacky, they'll need to bring a real live person to testify as to the debt, and if all that person can say is "that's what was on my computer screen," you should be able to get that thrown out.
