For the ongoing debate on the black list. In the AMEX membership agreement, it says that an additional card can be issued for anyone that has not had a "previous unsatisfactory account with AMEX" Whether anything negative (i.e. cancellation without charge-off) is considered "unsatisfactory", or if they will deny only real bad cases is unclear.
