Sometimes it is impossible to get a bill the moment you leave. A little over a year ago, I rushed my wife to the ER. Over 5 hours and a number of test later, they told her it was a kidney stone attack and gave her painkillers to get her to a doctors appointment the next day. It was after 10 P.M. when we left the ER. If I had wanted a bill, there was nobody there to provide one. I don't know if the hospital pulled her credit or not but this is one of those times where you just can pay when you leave even if you wanted too.
