CSC sent me a form to fill out. Photocopy of the following: SS# or Pay Stub with SS#, DL or Utility Bill or Lease Agreement I have been disputing with them since mid Jan, and have been making progress, 503 to 642. What are they up to? Is this a delay tactic?
