Let me add, mine is with Unum, which is now Unum-Provident. It is administered by Reinsure America (Swiss RE) - it has all been changed around since I got mine. Unum also merged with Paul Revere - so I guess the same, or similar products are available with all those companies.

I have a short term DI policy with Colonial Life and Accident, which has a short elimination period (waiting period), and then the larger DI policy with Unum, which has a 6 month elimination period and would pay me until age 65. Doing it that way saved me money on monthly premiums, while still giving me the amount of insurance I need.

Be sure you work with an agent who is experienced in disability insurance, because it has it's in-and-outs, depending on your occupation.

Breeze has it right, a short term "indemnity" type policy + a long term regular disabilty policy is the way to go.In addition to the other benefits, many of the short term policies pay IN ADDITION to workman's comp. and other insurance.
