FCRA violations and charge-offs

    Ok, I have a ton of questions but I'll start with these. =)

    I have been reviewing all three of my credit reports and have come across several FCRA violations. What is the procedure for dealing with these? Do I have to give the CRA's and CA's a chance to correct items before threatening suit? In other words can I get them to negotiate by telling them that they are in violation of the FCRA and I plan to file suit unless these are removed from my report? And who should this be directed to - the CRA, the creditor or both?

    Can one item be charged off multiple times? The same creditor as in problem #1 has the one account with the comment:
    Charge-off: 4/96, 9/95, 2/94
    Can they do that?

    Sorry so many questions. The more I research the more confused I get. Any help would be much appreciated.
