I know they all ask for copy of ID and whatnot, but what is really required for them to act on a dispute? Aside from name address and SSN as found in the sample letters is it necessary to sign it? What other documents in addition to the signature are required?
I (PERSONALLY) have NEVER provide any additional information other than NAME ADDRESS CITY STATE AND ZIP SS# AND D.O.B.

Re: Is sig/ID required for CRA disp I always sign my letters, whatever they are, to CRAs and CAs. The only exception to that is an online dispute. if they can accept an online dispute with no signature, then I see no reason why they can't accept a written dispute without a sig if you're providing enough info for them to verify who you are.

Re: Is sig/ID required for CRA disp I always include a report number - authenticates me and overomes any problems. Also avoids having my SS# on one more piece of paper floating out there.
