This identity theft nightmare is nearly over, I faxed a Knockout letter to Cross Country bank today about 1 of 2 bogus accounts on my credit report for a 200 dollar amount that was not mine,, I paid it anyhow when a bill accidentally came to my house, well 3 hrs after i faxed it i got a call from ACs presidents office and he left a voice mail saying he got the fax, he was very sorry, and the UDF is being sent to the CRA's for a deletion, Only 1 more deletion left, then im clear, but its a biggie, its for another CCB account that i realized was not mine due to the huge amt,, 800 dollars that has been assigned to collections, i did not mention this in the fax because im not paying a 800 dollar bill thats not mine, any suggestions on this last deletion..

Yes like an idiot i paid a bill that showed up at my house in a CC bill envelope, it looked nearly identical to my MBNA bill and the balance was within a dollar or two. so i figured ok they sold the account, i will pay it and close it.. umm i wont be doing that anymore

Finally, I just did a search for keywords ["knockout AND letter" -- without the quotes but include the all-caps "AND"] and came up with a few threads that may be of interest as well. The version you have linked above is the word for word exact same letter i used.. it will be interesting to see if those idiots at CCB actually send me a UDF like they said they would..
