Question: Does getting a letter from an attorney who was retained by the original creditor mean that a law suit is impending?

A couple of days ago I received a letter from an attorney trying to collect a 4 year old medical debt. His wording was pretty specific, and he said if I didn't respond within a week, he'd have the AUTHORITY to file a law suit. He didn't say he necessarily would file.

I'm wondering if this is just a scare tactic, or is this usual protocl before litigation?
