Newbie w/Questions

Hi everyone,
I am new to the board and have found all your post to be very helpful.

Here is my question.

I filed for BK in 2001 and it was discharged on 11/01
In an effort to clear my credit, I ordered my credit report from MYFICO.COM and found:

1. Several inquiries which I have not made. What letter should I send to get these removed?

2. I found that Equafax has the same debt twice and is counted as a negative account. What kind of letter should I send to clear that up?

3. Found that a bank has a $3,000.00 listed as a charge off when it was actually part of the bankruptcy, and has charge off/collection/bankruptcy as of 6/04 resulting in 26 90+ days delinquent.

4. I also notice that one of my mortgages that was discharged w/BK has BK on 12/01 then reports days late until 3/03 and then shows Forclosure/Voluntary Surrender on 4/03. What's up with that?

BTW- Iam in NJ and would greatly appreciate your advice.
