Well i've used with other banks. It's a real hassle to fill out a ton of forms. You have to provide: affidavit that item was lost/damaged proof of purchase (receipt, not charge) your insurance company (so they can call up and talk to them, make sure no double claiming , etc) police report Make sure you get all of the forms, certified mail , to them within the time frame they request.
